Notice regarding the Yokohama National University 2021 School Year Regular Health Exam

Yokohama National University
A Regular Health Exam will be given for all students (excluding credited auditors and auditors) as follows. For the 2021 school year, the health exams will be divided into on-campus health exams and off-campus health exams as a measure to prevent the spread of COVID-19. We appreciate your understanding and cooperation.
On-campus Health Exams
1. Target students
(1) New students entering in the 2021 school year (first-year college and graduate school students and first-year Tokyo Gakugei University united graduate school of education students enrolled at YNU)
(2) Students who are scheduled to undertake practicums as part of the university curriculum and need to take a health exam
(Second- and third-year students in the College of Education who are scheduled to undertake education practicums)
(3) Students who were enrolled in the 2020 school year (currently second-year college or graduate school students) who did not take the 2020 health exam.

3.Health exam details
Filling in a medical questionnaire, measurement of height and weight, vision test, hearing test (by interview), measurement of blood pressure, chest X-ray test, and medical check by doctors
4. Dates
April 7 (Wed), 8 (Thu), 9 (Fri), 12 (Mon), 13 (Tue), 14 (Wed), 15 (Thu), 16 (Fri), 19 (Mon), and 20 (Tue), 2021
● As an infection prevention measure, exam dates will be assigned. We will determine the time and number of people according to your school/college department,gender and birthday month. Your understanding is appreciated.

● You must have the following with you when you take the exam. Please have them ready.

If you experienced fever or were in poor physical condition during the period from one week prior to taking the health exam to the day of your exam, you will not be able to take the exam. Additionally, please avoid visiting 3C settings (closed spaces with poor ventilation, crowded and close-contact settings), dining in large groups, or coming in contact with individuals who may be infected two weeks prior to the exam
● If you have a class scheduled during your assigned health exam time, please prioritize your class.
● If you are unable to take the health exam at your assigned time, you may take the off-campus health exam provided free-of-charge by the Kanagawa Sogo Kenshin Center .
Students other than those targeted under categories (1) to (3) in the on-campus health exam.

April 2021 to March 2022
5. How to take the health exam
● You must make a reservation by telephone in advance (046-262-8155). We recommend that you make your reservation early because the health exams may be booked up ahead of time. (Free health exam)
● If you experienced fever or were in poor physical condition during the period from two weeks prior to taking the health exam to the day of your exam, you will not be able to take the exam.
● You must take your student ID card with you on the day of the health exam.
● The timing and place to pick up your health exam results (Individual Results Form) are as follows:
Timing: Four to five weeks after the health exam
Place: Administration department of your college or graduate school
● The results will be delivered in the form of an Individual Results Form addressed to the person who took the exam. The Kanagawa Sogo Kenshin Center can also issue a Health Examination Certificate for a fee.
● The above details may change depending on the COVID-19 infection situation.
● This exam is not mandatory if you live far away, feel anxious about taking a health exam on campus (due to an underlying condition, pregnancy, etc.), or are an adult student who has an opportunity to take a health exam other than the campus health exam, etc.
● During the period from two weeks prior to taking the health exam to the day of your exam, please avoid visiting 3C settings (closed spaces with poor ventilation, crowded and close-contact settings), dining in large groups, or coming in contact with individuals who may be infected.
2. Other notes
● If you take a health exam at another medical institution, please submit the results to the University (administration department of your college or graduate school).
● A Health Examination Certificate will be issued by the automatic certificate issuing system according to the following schedule:
● Students who took the 2019 school year on-campus health exam: Until March 31, 2021 (special measure)
● Students who will take an on-campus health exam in April 2021: From the end of May 2021 (scheduled)
● If you have a class scheduled during your assigned health exam time, please prioritize your class.
