Dear Students

Yokohama National University

Center for Health Service Sciences (Health Service Center)

　The Regular Health Exam for the 2021 school year was divided into on-campus health exams (already completed) and off-campus health exams (currently underway) as a measure to prevent the spread of COVID-19. The Regular Health Exam results (Individual Results Forms) will be delivered as detailed below. We ask for your understanding and cooperation regarding this matter.

For both on-campus and off-campus health exams

1. Delivery location
   Administration department of your college or graduate school
   Note: You must bring your student ID card with you when you pick up your health exam results. In addition, please tell the office whether you took the on-campus or off-campus health exam.


2. Document delivered
   The document will be an Individual Results Form which notifies the person undergoing the health exam of the results.
   Note: This is not the same as a Health Examination Certificate.

Students who underwent an on-campus health exam (Already completed between April 7 to 20, 2021 at the Education and Culture Hall)

1. Target students
   ①New students entering in the 2021 school year (first-year college and graduate school students and first-year Tokyo Gakugei University united graduate school of education students enrolled at YNU)
   ②Students who are scheduled to undertake practicums as part of the university curriculum and need to take a health exam(Second- and third-year students in the College of Education who are scheduled to undertake education practicums)
   ③Students who were enrolled in the 2020 school year (currently second-year college or graduate school students)who did not take the 2020 health exam


2. Delivery period
   From Tuesday, June 1, 2021


3. Health Examination Certificates
   A Health Examination Certificate can be issued by the automatic certificate issuing system from Tuesday, June 1, 2021.

Students who undertake an off-campus health exam (underway until March 2022)

1. Target students
   ①Students other than those targeted under the on-campus health exam
   ②Students targeted by the on-campus health exam who could not attend their scheduled health exam.
   
   Note: Students who will undertake the exam are asked to check the details here .


2. Delivery period
   Four to five weeks after the health exam


3. Health Examination Certificates
   Health Examination Certificates will be issued by the automatic certificate issuing system according to the following schedule. The data will be updated every two months.

　If you need a Health Examination Certificate prior to the scheduled starting date for the issuing of the certificates, a certificate can be issued by the Kanagawa Sogo Kenshin Center (location of health exam) for a fee. Please inquire directly with the Kanagawa Labor Health and Welfare Association (046-262-8155) when making a reservation.
　In addition, depending on who you will submit the document to, it may be possible to submit a copy of the Individual Results Form instead of a Health Examination Certificate. Please make inquiries and check the details yourself.

Other

• If you take a health exam at another medical institution, please submit the results to the University (administration department of your college or graduate school).


• Period for issuing Health Examination Certificates for 2021 academic year health exams using the automatic　certificate issuing system
  　We plan to make enable the issuing of certificates until May 2022
